Consent to the processing of personal data

I hereby, hereinafter referred to as "I, the Subject of Personal Data", in compliance with the requirements of the Federal Law of 27.07.2006 No. 152-FZ "On Personal Data" voluntarily, of my own free will and in my interests give my consent to Arplastik LLC (hereinafter referred to as the Operator) (OGRN 1111840003560), located at 426039, Udmurt Republic, Izhevsk, Votkinskoe shosse, bldg. 168a, office 2, to process my personal data specified by filling out the web form on the Operator's website eng.arplastik.ru and its subdomains (hereinafter referred to as the Site).

The purpose of processing personal data is to promote the Operator's goods, works, services on the market.

The volume of personal data to be processed: last name, first name, email address, telephone number, position, place of work, information collected through metric systems.

The processing of personal data means the collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, transfer (including provision, access), including cross-border transfer, depersonalization, blocking, deletion, destruction of my personal data) in documentary and electronic form. The processing of the personal data of the Subject of Personal Data can be carried out using automation tools and / or without the use of automation tools.

The date of receipt of consent to the processing of personal data of the Subject of Personal Data is the date of sending the web form from the Operator's Website. Consent is valid from the date of sending the web form from the Operator's Website and until the purpose of processing my personal data is achieved, unless it has been revoked by me in accordance with Art. 9 of the Federal Law of 27.07.2006 No. 152-FZ "On Personal Data".

This Consent may be revoked by me by submitting a written request to the above-mentioned address of the authorized representative of the Operator. The said written request must contain the following information about me: (1) last name, first name, patronymic; (2) address of residence; (3) name and number of the main identity document; (4) information about the date of issue of the said document and the body that issued it; (5) signature. In the event of receiving such a request, the Operator shall cease processing the personal data and destroy them within a period not exceeding 60 days from the date of receipt of the revocation (except for data subject to processing by the Operator in accordance with the requirements of the Russian Federation legislation). In this case, the Operator has the right, both in the event of my revocation of this Consent and after the expiration of this Consent, to continue processing my personal data to the extent that consent is not or will not be required for its implementation by virtue of the current legislation of the Russian Federation (including the grounds specified in paragraphs 2–11 of Part 1 of Article 6, Part 2 of Article 10 of the Federal Law of 27.07.2006 No. 152-FZ "On Personal Data").

I have read the procedure and possible consequences of revocation of this Consent.
